mioIDのご入力を誤ると回答特典を付与することができなくなりますため、お間違えの無いようご入力ください。

IIJmioモバイルサービスをご利用でないお客様は特典付与の対象外となりますのでご了承ください。

回答特典の抽選は、本アンケートに最後までご回答いただいた方を対象に行います。

回答特典の付与は、回答特典対象のお客様のご登録メールアドレス宛てにご案内いたします。(2018年9月上旬頃)
